The latest version of the Internal Routing Form for proposals is available on ORA’s website:
UMD/ORA Forms
Its use is effective immediately and is to be used for all proposals submitted to ORA. Please discontinue the use of previous versions of the Routing Form unless the form and proposal are currently being routed.
Key changes to the routing form include updates to question 35 to address mandatory PHS Financial Conflict of Interest policies and procedures
